IRS Returns

• Call 800-829-1040. Follow the prompts by pressing 1, then 2, then 1. Ask for a free transcript.
• You may also retrieve your returns at http://www.irs.gov/Individuals/Get-Transcript. This link takes you to a page where you can create an account. Once logged in to your account, you are able to view and print your tax transcripts.

Ohio Returns

• To receive a summary transcript of your Ohio income tax return, please register or log in to Online Services at http://www.tax.ohio.gov/ohio_individual/individual/filefaster.aspx. You may access your transcripts in the Tax Returns on File section of the Dashboard, which is displayed on the first page after logging in.
• You may also call Ohio toll-free at 1-800-282-1782 to receive a transcript. Please indicate the taxable year you are requesting, your social security number, and your current address. If you filed a joint Ohio income tax return, you must also indicate your spouse's social security number. The summary transcript will be mailed within three business days of your telephone request.

Kentucky Returns

• You may request transcripts via phone by calling 502-564-4581.

You must physically visit one of the following offices and request a copy of your recorded deed and any recorded mortgages. Someone at the Information Desk will print these for you. There is a small fee for these documents.
